What to bring to a GLP-1 follow-up appointment: a one-page summary from your spreadsheet
This guide is about keeping records, not medical advice. Your dose, schedule and injection technique are set by your prescriber and the patient leaflet; bring questions to them.
Follow-up appointments are short. A one-page summary helps you use the time for questions instead of trying to remember dates.
What to put on the page
- Current dose and the date it started.
- Dose history: each change and its date.
- Weight: start, now (7-day average) and the change.
- Side effects: which ones, how often in the last four weeks.
- Missed or late doses since the last visit.
- Your questions, written down.
Formulas that fill it in
Current dose (last row of the dose log):
=INDEX(Doses!D:D, COUNTA(Doses!D:D))
Side-effect count in the last 28 days, if your daily log has a column per symptom with a 1 when it happened:
=SUMIFS(Daily!C:C, Daily!A:A, ">="&TODAY()-28)
Days since last visit, with the visit date in a settings cell:
=TODAY()-Settings!B5
Print or share
Set the print area to the summary only (Page Layout → Print Area) and fit it to one page. In Google Sheets, use File → Download → PDF with "Selected cells". Bring it on paper or on your phone.
Let your doctor interpret the numbers. The sheet is there so nothing is forgotten.
Rather start with a ready-made file?
The GLP-1 Tracker has a doctor-visit summary sheet that fills in from your logs. Your data stays in your own file.
