Injection site rotation log: remember where you injected last week
An injection site rotation log is a simple record of which body area and side you used for each injection, so you can follow your leaflet’s instructions about changing spots without having to remember. With a weekly pen, it is easy to forget where last week’s injection went.

Important: this page is about record-keeping only. It does not give medical or dosing advice. Always follow the plan your prescriber or pharmacist gave you, and the leaflet that comes with your medicine.
What to record for each injection
- Date and time of the injection.
- Area: the areas named in your medicine’s leaflet, often stomach, thigh or upper arm.
- Side and spot: left or right, and for example upper or lower.
- Notes: anything you noticed at the site, such as redness or bruising, so you can mention it to your pharmacist or prescriber.
A simple layout
One row per injection with columns for date, area, side, spot and notes. Add a column with a formula that shows the last site used, so you see it before your next dose:
=INDEX(C:C,COUNTA(C:C))
Some people prefer a fixed weekly cycle, for example stomach left, stomach right, thigh left, thigh right. Only use areas your leaflet or prescriber says are suitable.
Why it helps
Your leaflet will tell you how to choose and change injection spots. A log removes the guesswork, makes it easy to notice if one area keeps reacting, and gives you something concrete to show if you have questions at an appointment.
Combine it with the rest of your log
Site rotation is one part of tracking GLP-1 treatment. Doses, weigh-ins and side effects belong in the same file; see our guide to tracking GLP-1 medication, the side effects diary and the Mounjaro tracker guide.
Ready-made
The GLP-1 Medication & Weight Loss Tracker logs the site for every injection next to the dose, so the last site is always visible. The free Lite version covers doses and weight.
FAQ
Which injection sites should I use?
Use the areas described in your medicine’s leaflet or by your prescriber. The log only records what you chose.
Does it work for daily injections too?
Yes. Add one row per injection; the last-site formula works the same.
Is it medical advice?
No. It is a record-keeping tool. Ask your pharmacist or prescriber about anything to do with injecting.
